Inspector Notes

This inspection was conducted by licensing staff using and alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on October 6, 2021 and concluded on October 19, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 8 children present directly supervised by 3 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The children were observed individual activates. The inspector reviewed on 2 children records and 1 staff record.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-100-A
Based on child record review, the provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 (date of enrollment 6/29/2021) did o have documentation of a physical examination avaialble for review during the inspection.
Plan of Correction: Physical Examination was obtained on October 22, 2021.
