Inspector Notes
An unannounced monitoring inspection was conducted May 1, 2024, between the hours of 10:40am - 1:30pm. There were 12 children (37 points) directly supervised by 2 caregivers. The home, 2 caregiver records, 5 children?s records, 1 household member record, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely and watching a show on the television. There was an assortment of age-appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

Violations
4Evidence:
1) A landline telephone was not observed in the family day home.
2) Caregiver #1 stated that there is not a landline telephone in the home.
1. Children from birth through 15 months of age count as four points each;
2. Children from 16 months through 23 months of age count as three points each;
3. Children from two through four years of age count as two points each;
4. Children from five years through nine years of age count as one point each; and
5. Children who are 10 years of age and older count as zero points.
Evidence:
1) At the beginning of the inspection there were 12 children present with two caregivers. The point total for the 12 children was 37 points, 5 points over the supervision guideline.
Evidence:
1)The record for Child #1 did not contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.
2) The record for Child #1 contained one name and one telephone number to contact in case of an emergency.
3) The record for Child #3 did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
Evidence:
1) Child #2 (age 32 months) most recent immunization documentation was dated 03/24/2022.
2) Child #3 (age 16 months) most recent immunization documentation was dated 08/03/2023.
3) Child #5 (age 20 months) most recent immunization documentation was dated 11/01/2022.
