A monitoring inspection in response to a self reported incident was conducted on 8/10/2022 . The inspector reviewed compliance in the area of staffing and supervision. One staff record was reviewed. Bottle feeding procedures were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards and a violation was documented on the violation notice issued to the program and discussed with program director during the exit interview.

Based upon staff interview, the facility has not ensured that when staff are supervising children, they ensure their care and protection.
Evidence:
Interviews with staff 2,3 and 4, on duty in the young infant room on 8/5/2022, confirmed that on 8/5/2022, staff 2 mistakenly fed child 1a bottle of breast milk that was labeled and intended for child 2.

The incident was immediately reported to both parents and to the Virginia Department of Education. All staff have been re-trained. Procedures have been updated and will be strictly enforced and documented.
