Inspector Notes

A renewal inspection was conducted on 10/21/2022. There were 109 children present, ranging in ages from infants to 5 years, with 20 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 7 child records and 9 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Based upon review of children's injury records, the facility has not ensured that all required documentation is included.
Evidence:
The injury record for child 8, dated 10/19/2022, did not include the date the parent was notified of the child's injury.
Plan of Correction: The facility responded with the following during the exit interview:
The parent of child 8 was notified of the child's injury and signed the injury record. All children's injury records will be carefully checked to ensure that all required documentation is complete.
