Inspector Notes
An unannounced monitoring inspection was initiated and completed on 10/29/2024 in response to a self-report received by the licensing office on 10/24/2024 relating to supervision of children. This inspection included:
? document review,
? tour of the facility, and
? interviews
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 4, 2024. A POC submitted after this date will not appear on the public website.
Violations
2On 10/24/2024, two year old child 1 was left alone washing their hands in the classroom bathroom for approximately two to four minutes before staff discovered that child 1 was not with the remainder of the children who had walked outside to the playground.
Teachers have been re-trained on our headcount name-to-face procedures through video and policy acknowledgements. The miscommunication happened because they were moving the class in small groups and need extra care when class is divided and make sure last teachers sweeps classroom before leaving.
On 10/24/2024, two year old child 1 was not within sight and sound supervision of staff when left alone in the classroom bathroom for approximately two to four minutes while the remainder of the children and the staff had walked outside to the playground.
