During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 01/09/2025 and completed on 01/16/2025 with local agencies, in response to a complaint received by the licensing office on 12/27/2025 relating to the allegation of forbidden action. The inspector reviewed 1 staff record on-site. This inspection included:
? document review,
? tour of the facility,
? interviews, and
? observation.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with an applicable standard or law, and the violation is documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. The following action or threat thereof is forbidden: restricting movement through binding or tying or forcing a child to assume an uncomfortable position.
In response to child 1 (three years old) not wanting to stay on the cot at nap time, Staff 1 restricted child 1's movement by placing their knee on child 1's back as child 1 lay face down on the cot.
Plan of Correction: The facility responded with the following:
We are appealing this violation. Admin saw child laughing and smiling on her cot. For utmost precaution, the staff member in question did not work alone while being investigated and was assigned training.
