During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/21/2025, as a part of the licensure period. The on-site inspection began at 12:00 PM and ended at 4:15 PM. The inspector reviewed compliance in the areas listed above. There were 133 children present and 21staff. the inspector reviewed nine children's records and 10 staff records on site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening.
The record provided for staff 9 included a tuberculosis test but no documentation that the test was read to be negative.
Plan of Correction: Facility response:
The staff person was contacted and is having the physician fax the results of the tuberculosis screening to us. In the future, we will carefully review the tuberculosis screenings to ensure the results are complete.
Standard 8VAC20-780-500-A
Staff shall wash their hands with soap and running water before and after a diaper change.
Staff 7 was observed removing her gloves and not washing her hands after changing a diaper.
Plan of Correction: Facility response:
All staff will be reminded that hand washing is required regardless of whether gloves are worn when changing diapers,

Disposable diapers shall eb disposed in a leakproof or plastic lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches the exterior surface of the storage system during disposal.
Staff 7 was observed opening the lid of the diaper disposal system and stuffing the soiled diaper inside.
Plan of Correction: Facility response:
Diaper changing procedures will be reviewed with all staff. the disposal system will be checked to ensure that diapers can be easily disposed using the foot operated step down mechanism.
