During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation
Inspector Notes
An unannounced monitoring inspection was initiated on and completed on 10/21/2025 in response to a self-report received by the licensing office on 10/20/2025 relating to Supervision. The inspector reviewed 1 child?s record and 2 staff records on-site. This inspection included:
? document review,
? tour of the facility,
? interviews, and
? observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, a violation that was not reported was found related to following policies.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on October 30, 2025. A POC submitted after this date will not appear on the public website
Violations
3Following morning outdoor play, staff 1 and 2 brought their class into the building, leaving two year old child 1 alone on the playground for approximately two to four minutes. The child was discovered by another staff who was bringing her class out to the playground.
Staff completed re-training on name to face procedures to ensure accurate counting of students when moving locations.
All classes are now required to call the office every time they come in from the playground for admin to ensure proper name to face procedures and to clear the playground.
Two year old child 1 was left alone on the playground out of sight and sound supervision of staff for approximately two to four minutes when staff 1 and 2 brought the rest of the children into the building.
Staff completed re-training on name to face procedures to ensure accurate counting of students when moving locations.
All classes are now required to call the office every time they come in from the playground for admin to ensure proper name to face procedures and to clear the playground.
Staff 1 and 2 did not follow facility procedures regarding accounting for children using name to face recognition checks when supervising children when they entered the building following outdoor play, leaving two year old child 1 on the playground.
