During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on October 23, 2024 in response to a complaint received by the licensing office on October 10, 2024 relating to allegations of 8VAC20-780- Staffing and Supervision, Special Care Provisions and Emergencies. and Special Services. The inspector reviewed (1) children?s record and (1) staff record on site.This inspection included documentation review, interviews and observation.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 6, 2024. A POC submitted after this date will not appear on the public website.

Then center is required to maintain a written record of children's serious or minor injuries on the day of the occurrence. The record shall include the date and time of injury; name of injured child; type of circumstance of the injury; staff present and treatment; date and time parents were notified; any future action to prevent recurrence of the injury; two signatures, and documentation of how parent was notified. The center did not have written documentation for Child #1.
Plan of Correction: The rationale for this standard is to ensure the parent is notified of an injury occurring at the center. Although a message was immediately sent to the parent with a an accompanying photo, and the child was evaluated by the Director, and a phone conference was held between Directors and parent regarding the details surrounding the scratch, we did not use the written form because the scratch was
unknown to have occurred at school. We use an electronic written record for injuries occurring at the center, but we will now include all injuries, regardless of their origin, unless the parent identifies the injury in advance of care.
