During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 2/7/2025, as a part of the licensure period. The on-site inspection began at 9:55 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 21 children present and 7 staff. The inspector reviewed 3 children?s records and 3 staff records [on-site]
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include documentation of how parent was notified, date and time when parents were notified and any future action to prevent recurrence of injury.

Seven of seven records reviewed were incomplete and missing documentation of how parent was notified, date and time when parents were notified and any future action to prevent recurrence of injury.
Plan of Correction: Violation #1: All the incident and accident reports have been completed with all information to include how the parent was notified, when they were notified and future actions to avoid any recurrence of injury. Staff have been retrained on how to fill out an incident and accident report to ensure that all areas are completed and documented appropriately.
