8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Inspector Notes

An unannounced subsidy inspection was conducted today between 10am and 11:40am. . There were a total of 4 children present (two belonging to the Vendor) = 11 points with the Vendor at the time of the inspection. Ratios and supervision were in compliance. There was also an adult household member present during the inspection. The daycare was sufficiently supplied with toys and equipment for children, and appeared clean and organized today. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Based on child record review, 2 child records did not contain all required information.
Evidence:
1. Child #1 and #2's records did not contain a name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reachedan emergency contact.
2. Child #1 and #2's records did not contain a written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately
Plan of Correction: The Vendor will obtain the emergency contact and medical care authorization from the parent this afternoon.
Standard 8VAC20-790-260-K
Based on observation of the Vendor's home, one area of the home, stairs were accessible to children under the age of 2 years.
Evidence:
The stairs leading to the middle level of the home did not have a gate/barrier and was accessible to two children under the age of 2 years.
Plan of Correction: The gate was upstairs. The gate will be placed at the steps to prevent access to children.
