Inspector Notes

An unannounced complaint inspection was conducted on 2/26/24 in response to a complaint that was received by the Virginia Department of Education, Office of Child Care Health and Safety. Interviews were conducted with staff relating to allegations of physical abuse, behavior guidance, following injury procedures, and lack of injury reports. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegations of lack of injury reports, and the complaint is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days. Standard 8VAC20-780-40-I
Based on staff interviews, the facility did not ensure that they followed their own policies around head injuries and injury reports.
Evidence: staff confirmed during their interviews that the school has policies on head injuries and injury reports, and that on 2/14/24 they did not notify parent(s) of the children's head injury nor did they complete an injury report. Staff have reprimands in their files for not following school policies for injuries.

Based on review, the facility did not ensure that they maintained a written record of children's serious and minor injuries.
Evidence:
1) the facility failed to document an child's head injury that occurred between two children on 2/14/24 (children ran into each other and hit their heads leaving a small mark).
2) of the 20 injury reports reviewed, 18 did not have documentation of the time/date parents were notified.
3) of the 20 injury reports reviewed,11 did not have documentation of prevention of future occurrence.
