During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on May 20, 2025 and completed on May 20, 2025. The on-site inspection began at 12:53 pm and ended at 2:10 pm. The inspector reviewed compliance in the areas listed above. There were 101 children present and 20 staff. The inspector reviewed 8 children?s records and 3 staff records on-site on mAY 20, 2025.

This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 3, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations is required once every six months for children under the age of two years. 'Child #2, 23 months old, last documented immunization was over 12 months ago.
Plan of Correction: Called family on 5/20/25 and requested to send the updated immunization report.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Medication for Child #4 was onsite, but the medication authorization had expired over
5 months ago. The medication had not been administered past the expiration of the authorization. The authorization had not been renewed and the medication was not returned or disposed of as required.
Plan of Correction: We had communication with family, they mentioned, no longer need the medication
but missed to return the me to home. We return the medication on 5/20/25
during pickup time and request with family to have doctor notes mentioning
no longer needs medication
