During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated on March 5, 2026 and completed on April 10, 2026 with a local agency in response to a complaint received by the licensing office on March 5, 2026, relating to allegations of behavioral guidance. The inspector reviewed 2 children?s records and 3 staff records on-site on March 5, 2026. This inspection included document review, tour of the facility, observations, and interviews.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to programs and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 17, 2026. A POC submitted after this date will not appear on the public website.

On February 27, 2026, in the Dolphins classroom, video camera recording showed three children were on lying down on cots for naptime. The three cots were not at least 12 inches of space between each cot.
Plan of Correction: Reminded all the staffs again to place the cots at least 12 inches apart.
