Inspector Notes

An unannounced monitoring inspection was conducted 5/1/2024 from 10:30am-12:05pm. There were 11 children (24 points) directly supervised by 2 caregivers. The home, 2 caregiver records, 3 children?s records, 1 household member?s record and pet records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely dancing to music, and having a lesson on body parts, colors and clothing. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on observation and interview, a wood burning fireplace and associated chimney was not inspected annually.
Evidence: Provider did not have a fireplace inspection.
Plan of Correction: The provider will contact/schedule fireplace inspection.
Standard 8VAC20-800-780-B
Based on record review and interview, the provider did not obtain parent's signed acknowledgment of annual review of emergency contact information and the
parent's written authorization for emergency medical care.
Evidence: Child #1 (1st day 3/1/22) file did not contain signature/date of annual review of emergency information.
Plan of Correction: The provider will have parents review, update and sign emergency contact information, annually.
Standard 8VAC20-800-90-C
Based on record review, the provider did not obtain documentation of additional immunizations per guidelines.
Evidence Child #2 (DOB: 7/2020, last immunization dated 2/2022) and Child #3 (DOB: 10/2021, last immunization 1/2023).
Plan of Correction: The provider will request update immunization records from parent.

Based on record review, interview and observation the provider did not obtain written permission to go off premises.
Evidence:
1. The provider and children were not on premises at arrival of inspector
2. Children's records did not contain and provider acknowledged written permission from parents to be taken off of the premises had not been obtained.
