During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 03/17/2025 and completed on 03/18/2025, in response to a complaint received by the licensing office on 03/17/2025 relating to allegations of supervision. The inspector reviewed 19 children?s records and 5 staff/household member files on site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/26/2025. A POC submitted after this date will not appear on the public website.
Violations
7Poisonous cleaning chemicals were on the back of the toilet, on a low shelf in the hallway, and in a wet mop dispenser in the play area.
The provider's license allows for a capacity of 12, with a zoning stipulation that includes any resident children be included in that capacity. The provider stated there are times when the resident children are with the children in the family day home, totaling 14 children.
There were attendance records on the date of inspection for the current and previous day of care. The provider did not take written attendance during the time period of February 1, 2025 - March 14, 2025.
Child #9 was sleeping in a rocking infant seat in the play area from 1:00 pm to 1:20 pm during the inspection.
The family day home did not have last dates of attendance listed for child #16, child #17, child #18, and child #19.
Child #3 was in a high chair for 37 minutes without being moved.
During the inspection, the provider stated lunch time was at 1:00 pm, the schedule on the wall listed lunch time at 12:30 pm, and the previous meal time was at approximately 10:00 am. As of 1:30 pm, at the end of the inspection, the children had still not eaten lunch.
