During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 03/17/2025 and completed on 03/18/2025, in response to a complaint received by the licensing office on 03/17/2025 relating to allegations of supervision. The inspector reviewed 19 children?s records and 5 staff/household member files on site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/26/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children.

Poisonous cleaning chemicals were on the back of the toilet, on a low shelf in the hallway, and in a wet mop dispenser in the play area.
Plan of Correction: All of our cleaning supplies will be put away from the reach of children, mop will be stored in the bathroom after use.
Standard 8VAC20-800-30-A
The provider shall ensure compliance with these standards and the terms of the current license issued by the department and with relevant federal, state or local laws, and other relevant regulations.

The provider's license allows for a capacity of 12, with a zoning stipulation that includes any resident children be included in that capacity. The provider stated there are times when the resident children are with the children in the family day home, totaling 14 children.
Plan of Correction: My own two kids will remain upstairs in my house under the care of my husband or their grandma and away from the daycare at all times. If they are outside, they will remain off daycare premises.
Standard 8VAC20-800-50-A
The family day home shall keep a written record of children in attendance each day.

There were attendance records on the date of inspection for the current and previous day of care. The provider did not take written attendance during the time period of February 1, 2025 - March 14, 2025.
Plan of Correction: Starting today I will make sure to keep a file of all of my attendance sheets. We will have an extra when we are outside and will keep all attendance for two months.
Standard 8VAC20-800-590-B
An infant child who falls asleep in a play space other than his own crib, shall be moved promptly to his designated sleeping space.

Child #9 was sleeping in a rocking infant seat in the play area from 1:00 pm to 1:20 pm during the inspection.
Plan of Correction: Any kid who falls asleep in any other space other than their crib will be moved immediately to their crib.
Standard 8VAC20-800-60-B
A child's record shall contain first and last dates of attendance.

The family day home did not have last dates of attendance listed for child #16, child #17, child #18, and child #19.
Plan of Correction: Starting today I will be on top of all the children paperwork. The records of the kids no longer attending will be dated and removed from the folder of current attendees.
Standard 8VAC20-800-600-B
Infants shall spend no more than 30 minutes of consecutive time during waking hours confined in a high chair.

Child #3 was in a high chair for 37 minutes without being moved.
Plan of Correction: I got nervous during the inspection, no other child has or will be ever left for more than 30 minutes in the same space.
Standard 8VAC20-800-870-C
The family day home shall schedule snacks and meals so there is no more than three hours between meal times.

During the inspection, the provider stated lunch time was at 1:00 pm, the schedule on the wall listed lunch time at 12:30 pm, and the previous meal time was at approximately 10:00 am. As of 1:30 pm, at the end of the inspection, the children had still not eaten lunch.
Plan of Correction: We will stick to our scheduled meals as planned regardless of any interruptions we might have.
