Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was initiated on 4/20/21 and concluded on 4/21/21. The Program Director was contacted by telephone to initiate the inspection. There were 86 children present and 6 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

1. The record for staff #1 did not contain documentation to demonstrate staff #1 had completed the Virginia Preservice Training for Child Care Staff.
2. The record for staff #3 did not contain documentation to demonstrate staff #1 had completed the Virginia Preservice Training for Child Care Staff.
3. Staff #5 (Program Director) reviewed the records for staff #1 and staff #3, and confirmed that none of the staff had completed the Virginia Preservice Training for Child Care Staff.
