A monitoring inspection was initiated on 6/30/22 after receiving a self-report from the facility in regards to compliance with the regulations that was received on 5/25/22. Staff were interviewed, the facility was inspected and documentation was reviewed during the inspection. The investigation was concluded on 4/6/22. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
