During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 4/2/25. The on-site inspection began at 11:05am and ended at 2:20pm. The inspector reviewed compliance in the areas listed above. There were 32 children present and five staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, a tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/14/25. A POC submitted after this date will not appear on the public website.

5The immunization record for Child #2 (enrolled for over a year) was not signed by a physician, his designee, or an official of a local health department, which is required.

Virginia Department of Education-sponsored orientation was completed.
There was a spray bottle of multi-purpose cleaner stored in an unlocked cabinet.
On the preschool playground and on the school age playground, three inches of resilient surfacing was measured, which does not meet the minimum requirement. Children were observed playing on the preschool playground during the inspection.
The record for Child #1 (enrolled for less than a week) did not contain the home address and phone number of one parent or previous child day care and schools attended by the child.
