During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated and completed on 12/16/25. in response to a self-report received by the licensing office on 11/17/25 relating to Attendance records; reports. The inspector reviewed one children?s records and two staff records on-site. This inspection included document review and interviews. Though not a part of the original self report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/4/26. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-2
Centers must obtain a completed national criminal background check prior to the employee's first day of employment.
The criminal background check for Staff #2 was not obtained prior to employment. The background checked was obtained three weeks after the staff member's hire date.
Plan of Correction: The program director will ensure that going forward staff will complete a criminal background check prior to employment for each new staff member.

Repeat Violation
The center must request a search of the central registry prior to the employee's first day of employment.
Staff #1 (employed fors even months) did not have a central registry search requested prior to the staff member's first day of employment. The central registry search was requested five months after hire.
Plan of Correction: The program director will ensure that going forward staff will complete a central registry search prior to employment for each new staff member.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
There was no documentation to show that an out of state central registry search or a sex offender check were requested for Staff #1 (employed for four months). The staff member was observed working on the day of the inspection.
Plan of Correction: The program director will ensure that going forward staff will complete an out of state background check at the time of employment.
Standard 8VAC20-780-160-A
Repeat Violation
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis (TB) screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.
The TB results for Staff #2 were not received at the time of employment. The results were received two weeks after the staff member's first day.
Plan of Correction: The program director will ensure that going forward staff will complete a TB screening prior to employment for each staff member.
Standard 8VAC20-780-70
Staff records shall be kept for each staff person and contain all the required information.
The record for Staff #2 did not contain documentation to show that two or more references as to character and reputation as well as competency were checked before employment.
Plan of Correction: The program director will ensure that two references will be completed prior to employment for each staff member.
