Inspector Notes
An unannounced Renewal inspection was conducted this date. A total of 10 children (24pts) with 2 staff were onsite. The children has just completed lunch and interacting with a video program that included singing. Observation of the programming area, backyard play area, crib and nap room was made. Walk thru of the entire home was not completed due to maintaining caregiver points. No medication needs at this time. The programming space was well maintained, toys and equipment within reach of the children which enhanced independence in selection of activities. A total of 4 children's records, all staff and household records were reviewed. This inspection began at approximately 10am and ended at 11:30am. Staff #2 did not complete the required 16 hours of annual training.
Evidence:
1. Child #1, #2 and #4's record did not contain the provider's medication policy.
2. Child #2 and #4's record did not contain parents' signed acknowledgement that parent information such as liability insurance, provisions of the emergency plan was received.
