Inspector Notes

An unannounced monitoring inspection was conducted 6/12/2024 between the hours of 10:05am through 11:00am. There were a total of 7 children present, ranging in age from 13 months to 4 years old (18 points) with the Provider and 1 assistant. A total of 5 children?s files, 2 staff files, and 2 household member?s files were reviewed. The children were engaged in the following: playing with magnatiles, doing puzzles, playing with play dough, and engaging in outdoor play. Positive interactions between caregivers and children were observed during this inspection. A complete inspection of the programming space, children and staff records, emergency drills, menu, attendance, and emergency procedures were completed today. No new staff have been hired since the last inspection. The provider advised there was no medication on site today. There was an adequate supply of age appropriate materials and toys. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not ensure children's records contained all required information.

Evidence: Child #1's record was missing parent's signed acknowledgement of the receipt of the Information for Parents form. Child #3's record was missing a second emergency contact name, address, and phone number. Child #5's record was missing the medical insurance policy number.
