Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 9/23/2024. The on-site inspection began at 11:05am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 caregivers; the point total was 24 points. The inspector reviewed 5 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the provider did not obtain an updated Tuberculosis (TB) screening for each caregiver and adult household member that was completed within the last two years.

Evidence: Staff #1's most recent TB screening was completed 2/22/22. Adult Household Member #1's most recent TB screening was completed 3/2/22. Adult Household Member #2's most recent TB screening was completed 2/23/22.
Plan of Correction: Everyone will obtain updated TB test.
