Standard 8VAC20-800-180-A
Based on review of one household member record, the provider did not obtain for each adult household member a current Report of Tuberculosis (TB) Screening form every two years from the date of the first screening. Evidence: The TB test on record for Household Member A was dated 1/1/2020.
Plan of Correction: I will remind the Household Member that this needs to be completed as soon as possible.
Standard 8VAC20-800-320-B
Based on observation, the provider did not ensure that child-resistant protective covers were installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Evidence: Four outlets were observed to be uncovered during the inspection and all were of a height to be accessible to children.
Plan of Correction: I have a large pack of outlet covers. I will find it and make sure the outlets are covered by the end of the day today.
Standard 8VAC20-800-690-D
Based on observation, the provider did not ensure that the diapering surface was nonabsorbent and washable. Evidence: The diaper changing pad had multiple tears in the nonabsorbent cover, making the surface absorbent and non-washable where the padding was exposed.
Plan of Correction: I will purchase a new changing pad today.
Standard 8VAC20-800-90-C
Based on review of four child records, the provider did not obtain documentation of additional immunizations for a child once every six months for children under the age of two years. Evidence: The most recent immunizations on record for Child A, with a date of birth of 11/1/2020, were dated 9/23/21.
Plan of Correction: I have asked the parents for this already. I will follow up with them again and remind all parents to please bring me immunization updates each time they get immunizations.
