During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 1/23/2025. The on-site inspection began at 10:20am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 16. The inspector reviewed 5 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/4/2025. A POC submitted after this date will not appear on the public website.

The family day home must obtain a completed national criminal background check prior to the employee's first day of employment.

Caregiver A?s fingerprinting results were dated over one month after their first day of employment.

The family day home shall obtain a copy of the results of a search of the central registry prior to an employee's first day of employment or request the central registry prior to employment as long as satisfactory national criminal background check results are obtained prior to employment.

Caregiver A has been actively employed for 4 months and the search of central registry has not been requested or received.

Any childcare program that serves preschool-age children shall be equipped with at least one carbon monoxide detector.

The home did not have a carbon monoxide detector on site at the time of the inspection.
Plan of Correction: Carbon monoxide detector was purchased and will be installed soon.

The provider shall obtain from each caregiver at the time of hire prior to coming into contact with children a current Report of Tuberculosis Screening form.

Caregiver A?s TB screening was dated 17 days after they began employment and came into contact with children.

Whenever the family day home has agreed to administer prescription medications or nonprescription medications, the administration shall be performed by a caregiver who has satisfactorily completed an approved training program for this purpose and is retrained at three-year intervals.

One child was present with emergency medication. Documentation shows the provider has agreed to administer nonprescription medications. Caregiver B?s MAT training expired over 2 years ago therefore there was no one currently trained to administer medication.
Plan of Correction: I will be taking MAT training class or renew. Provider does not administer medication.

(Repeat Violation) Potentially poisonous substances such as cleaning agents shall be stored away in areas inaccessible to children.

In the bathroom designated for child use is a spray bottle of antiseptic in an unlocked cabinet accessible to children.

(Repeat Violation) A caregiver shall provide direct care and supervision of each child at all times to include the monitoring of each sleeping infants.

1. A 12-month-old child was sleeping in a separate room on the same level with the door closed.
2. There was no baby monitor being used and caregivers did not do an in-person observation every 15 minutes.
Plan of Correction: We provide direct care and supervision of each child. Baby monitor was replaced and used after a few days.

When time out is used as a discipline technique it shall be used sparingly and shall not exceed one minute for each year of the child?s age.

Child A age 2 years was placed in time out for approximately 10 minutes.

Caregivers shall wash their hands with liquid soap and warm running water before preparing and serving food.

Caregiver A was observed serving food to children without washing their hands prior to serving.
Plan of Correction: Caregivers will wash hands before and after meals or snacks.

Caregivers shall ensure that children?s hands are washed with liquid soap and warm running water before eating and after diapering.

1. Children were observed receiving snack, but handwashing was not done prior to eating.
2. After diapering, a child?s hands were not washed after.
Plan of Correction: Children will wash hands before and after meals and snacks.

Medication shall be kept in a locked place using a safe locking method that prevents access by children.

2 medications were observed in an unlocked upper cabinet.
Plan of Correction: The lock was placed back on.

Annually, the provider shall review with the parent the emergency contact information and obtain the parents signed acknowledgment of the review.
1. The annual review of emergency information along with parental signature for Child A was overdue by a year and a half.
2. The annual review of emergency information along with parental signature for Child B was overdue by 3 years.
3. The annual review of emergency information along with parental signature for Child C was overdue by 3 years.
Plan of Correction: I will have a teacher work day to review all paperwork.
Standard 8VAC20-800-830-A
(Repeat Violation) Emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.

Emergency evacuation procedures were not practiced monthly. Monthly drills were not documented for July, August, September and October of 2024.
Plan of Correction: Emergency evacuation procedures were practiced monthly and documented on forms which were misplaced. Immediately document all and store in one place.
