Based on a review of two children's records, the provider did not ensure one child's record contained the required information.
Evidence: 1) The record for Child #1, enrolled on 10/19/20, contained the name and telephone number of two emergency contacts, but the record did not contain the address of each designated person. Each child record shal contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.
