Inspector Notes

An unannounced complaint investigation was conducted on 9/8/23 from 8:20am to 9:35am in response to a complaint received in the licensing office regarding allegations pertaining to supervision; ratios; behavioral guidance policies; care, protection, and guidance; and smoking on the premises. Based on review of documentation, observation, and interviews conducted, the allegation regarding care, protection, and guidance was determined to be valid. The allegations regarding supervision, ratios, behavioral guidance policies, and smoking on the premises were unable to be substantiated due to insufficient corroborating evidence. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 9/27/23. Based on interview, the center did not ensure that when staff were supervising children, they always ensured their care, protection, and guidance. Evidence: On 7/19/23 the Schoolage class went on a field trip to a water park. Staff did not have the children in one group reapply their sunscreen and approximately eight children received sunburns.
Plan of Correction: Staff will utilize a sunscreen log which records when sunscreen was applied to each student while on field trips. It will record application before the field trip, upon arrival to field trip, and then every 2 hours. Staff will document if student refused or did not have sunscreen.
