Inspector Notes

An unannounced complaint investigation was conducted with the assistant director on 6/12/24 from 9:35am to 11:20am in response to a complaint received in the licensing office regarding supervision, following policies, and documentation of injuries. Accident/incident reports were reviewed, interviews were conducted, policies and procedures were reviewed and discussed, and classroom observations were conducted. There were no violations cited based on the complaint allegations. One violation was found during the investigation that was not part of the complaint. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 6/21/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Based on review of documentation, the center did not maintain a written record of children?s serious and minor injuries that included any future action to prevent recurrence of the injury. Evidence: During the investigation, 98 accident/incident reports were reviewed. 66 of the accident/incident reports did not contain documentation of the actions the center took to prevent recurrence of the injury.
Plan of Correction: All reports were updated with required information. All teachers were trained on how to properly complete reports.
