An unannounced monitoring inspection was initiated on July 8, 2024 and completed on July 25, 2024, in response to a self-report received by the licensing office on June 27, 2024 relating to supervision. The inspector reviewed two children?s records and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
