During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 11/06/2024 and completed on 12/05/2024 in response to a complaint received by the licensing office on 10/31/2024 relating to allegations of supervision, injury reporting, parental involvement, toileting procedures, and preventing the spread of disease. The inspector reviewed one child?s record and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports parts of the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 01/23/2025. A POC submitted after this date will not appear on the public website.

When any surface has been contaminated with body fluids, it shall be cleaned and sanitized.

On October 28, Child #1's soiled bedding, clothes, and socks were placed on a table used for serving food after the child had a toileting accident. The clothes had not been removed and the area had not been cleaned or sanitized for over an hour.

Standard 8VAC20-780-500-B
The diapering area shall have a nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing.

The diapering area in Room 614, where two-year-old children receive care, did not have a changing table or countertop designated for changing.
