During the inspection, the inspector reviewed the areas listed to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any
serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 03/20/25 and completed on 05/12/25, in response to a complaint received by the licensing office on 03/18/25 relating to allegations of supervision and forbidden action by staff. The inspector reviewed one child?s record and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 05/20/25. A POC submitted after this date will not appear on the public website.

4The TB in the record of Staff #1 was obtained 20 days after employment. A previous TB in the record expired over a month prior to the staff's first day of employment.
Video footage reviewed shows that on 03/18/25, Staff #1 used physical methods to remove Child #1 (3-year-old) from a group activity when the child was disrupting the class. After multiple attempts to try to get Child #1 to stop disrupting the class, Staff #1 tried to get the child to move from the carpet to a spot next to the staff's chair. The staff picked up the child under her arms and said ?I don?t like to pick you up. I don?t like to pick you up. I don?t like to pick you up.? The child resisted by going limp and released one arm. The staff continued moving the child while holding only one arm, lifting her, and using a dragging motion until they were by the staff?s chair.
The center's discipline policies state "Inappropriate behavior is explained, and children are encouraged to be responsible for their own behavior. When minor incidents occur, our teachers redirect children to avoid further problems. At no time will a teacher use physical punishment or punishment related to food, bathroom or naps. Our teachers never ridicule, embarrass or humiliate children."
Video footage reviewed shows that on 03/18/25, Staff #1 used physical methods to move Child #1 (3-year-old) away from a group activity in an attempt to stop the child's disruptive behavior during class. Staff #1 tried to get the child to move from the carpet to a spot next to the staff's chair. The staff picked up the child under her arms and said ?I don?t like to pick
you up. I don?t like to pick you up. I don?t like to pick you up.? The child resisted by going limp
and released one arm. The staff continued moving the child while holding only one arm,
lifting her, and using a dragging motion until they were by the staff?s chair.
Staff #1's interactions with Child #1 were not constructive in nature and were not intended to redirect the child to appropriate behavior.
On 03/18/25, Staff #1 roughly removed Child #1 (3-year-old) from a group activity when the child was disrupting the class.
Video footage reviewed shows a child tell Staff #1 that Child #1 pulled their hair. Staff #1 asked the child to stop, but the child did not comply. Staff #1 got up from her chair, walked over to the child, and said ?I?m gonna need you to move now. That?s all done.? The child said, ?I?m not moving.? The staff then proceeded to pick up the child under her arms. The child resisted by going limp and released one arm. The staff continued moving the child while holding only one arm, lifting her, and using a dragging motion until they were by the staff?s chair. The distance between where the child was, and the staff?s chair was approximately 6 feet.
