During the inspection, the inspector reviewed the areas listed. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 05/13/25 and completed on 07/24/25, in response to a complaint received by the licensing office on 05/13/25 relating to allegations of supervision, equipment and materials, and toileting procedures. The inspector reviewed one child's record and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports part of the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 08/04/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
(Repeat Violation) When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 05/12/25 when Child #1 (two-year-old) was picked up, his diaper was soaked and soiled. A collateral interview reported the child?s ?butt was raw and bleeding? and the child screamed when his diaper was changed. Management acknowledged seeing a diaper rash and redness at the end of the day when it was brought to their attention. The child was visibly walking with discomfort and said ?ow? when he was carried. Staff interviewed reported Child #1 did not have a diaper rash in the morning or throughout the day, and reported the child?s last diaper change was between 2:45pm and 3:30pm. Child #1 was picked up at 4:52pm. The center failed to protect Child #1 by not ensuring he was promptly changed when his diaper became wet or soiled causing the child to have a diaper rash.
Plan of Correction: Per the Center: "Staff members were retrained on diapering procedures."

If diaper ointment or cream is used, written parent authorization noting any known adverse reactions shall be obtained.

A tube of diaper cream for Child #1 was observed, but no written parent authorization could be located at the time of the inspection. It was reported the cream had been applied to Child #1 for approximately three days.
Plan of Correction: Per the Center: "Form was submitted to parent to complete. Retrained staff; cream is not to be applied without completed form."
