Inspector Notes

An unannounced monitoring inspection was initiated on 07/24/2025 and completed on 07/28/2025, in response to a self-report received by the licensing office on 07/24/2025 relating to programs. The inspector reviewed one child's record and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to administration and staff qualifications and are documented on the violation notice issued to the program.

The center must request a search of the central registry prior to the employee's first day of employment.

The central registry search for Staff #1 was requested one day late.

Standard 8VAC20-780-160-A
(Repeat Violation) The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.

The TB screening for Staff #1 was dated one day after their first day of employment.

(Repeat Violation) The center is required to ensure the center's own policies and procedures that are required by these standards are in compliance.

On 07/23/25, Staff #1 did not follow the center?s discipline policies when Staff #1 physically moved Child #1 (3-year-old) from one area of the playground to another when the child did not want to wear his shoes. Documentation and video footage reviewed showed that after attempting to put shoes on Child #1, the staff picked up the child by one arm and proceeded to drag him approximately four feet before picking him up under his arms. Staff #1 took the child to the edge of the playground, dropped his shoes on the ground, and walked away.

The center's discipline policies state "Discipline will be constructive in nature and include the following techniques: Using limits that are fair, consistently applied, and appropriate and understandable for the child?s developmental level and age; providing children with reasons for limits; giving positively worded directions; modeling and redirecting children to acceptable behavior??

Standard 8VAC20-780-400-B
(Repeat Violation) Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Documentation and video footage reviewed showed that while on the playground on 07/23/25, after several attempts to put shoes on Child #1 (3-year-old), Staff #1 picked up the child by one arm and proceeded to drag him approximately four feet before picking him up under his arms. Staff #1 took the child to the edge of the playground, dropped his shoes on the ground, and walked away. The child was observed sitting in the same spot, with his shoes off for approximately five minutes.

Staff #1's interactions with Child #1 were not constructive in nature and were not intended to
redirect the child to appropriate behavior.
