During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

This inspection was amended and sent to provider on 08/18/2025.

An unannounced inspection was initiated on 08/05/2025 and completed on 08/12/2025, with local agencies, in response to a complaint received by the licensing office on 07/28/2025 relating to allegations of supervision and hazardous substances. The inspector reviewed three children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/25/2025. A POC submitted after this date will not appear on the public website.

Complaint Violation:
Cosmetics, medications, or other harmful agents shall not be stored in areas, purses or pockets that are accessible to children.

On 07/25/2025, staff #1 had a vape pen in their pocket while supervising children. Based on video surveillance, the vape pen dropped out of staff #1?s pocket where children found the vape pen on the floor and were in possession of the pen for approximately 35 minutes.
Plan of Correction: A no bag in classroom policy was implemented. Lockers were ordered for staff. Review of harmful substance policy for staff.

Complaint Violation & Repeat Violation:
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 07/25/2025, children in classroom #1 were in possession of a vape pen for approximately 35 minutes. Based on video surveillance, staff # 1 was observed sitting on the floor with the children. At approximately 12:39, staff #1 got up from the floor and a vape pen fell out of their pocket onto the ground. Child #3 found the vape pen on the floor and picked it up to put in their play purse. Child #1 and child #2 were also seen playing with the vape pen and tried to use the vape pen. At approximately 1:14, staff #5 found child #3 with a vape pen and took the device to management.
Plan of Correction: Assign trainings to all teachers on active supervision, in person training for active supervision request from VQB5 next inservice. Updating policies in handbook.
