Inspector Notes
An unannounced, on-site focused inspection was initiated on 02/20/2026 and completed on 02/20/2026, as a part of probation. The Department issued a Special Order on 09/04/2025 to place the licensee on probation for a period of six months and requiring the licensee to contact parents, guardians, or other responsible persons as a result of high-risk violations. The on-site inspection began at 11:40am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility, interviews, and observations.
