Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview and a virtual tour of the program.
A monitoring inspection was initiated on 08/06/2021 and concluded on 08/18/2021. The provider was contacted by telephone to initiate the inspection. There were 6 children present, 13 points, and one caregiver. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 caregiver record, along with any requested program records submitted by the provider to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2Evidence:
1.The most recent tuberculosis screening in the record of caregiver 1 is dated 04/26/2018 which is not within the past two years.
2. The provider verified that documentation of a more recent tuberculosis screening has not been obtained.
A repeat tuberculosis screening will be obtained.
Evidence:
The provider was unable to show documentation that the parent of child 1 was provided, in writing, the provider's decision regarding the administration of medication.
Each parent will be required to to sign the Decision Regarding the Administration of Medication form. This form will be updated in all children's records to indicate that no medication (prescription, topical or over-the-counter) will be administered to children in care.
