The center must ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment.
1. 3-4 year olds classroom: a. Peeling paint was found on the wall around the window, b. a broken plastic bin was found in the home living sink equipment.
2. Pre-K classroom: A broken plastic bin was found in the home living sink equipment.

Plan of Correction: The bins were removed during the inspection. The peeling paint will be corrected.

The center must submit a request for an out of state central registry and sex offender search for an employee that has resided outside of Virginia within the last 5 years, prior to the first day of hire.
1. Staff #1and Staff #2's records indicated that they have both resided outside of Virginia within the last 5 years.
2. The out of state central registry searches for both employees were not submitted prior to their hire dates. The results have not been obtained to this date.
3. The out of state sex offender search for Staff #2 was not obtained prior to the hire date.

Plan of Correction: The sex offender search was completed during the inspection. The out of state CRS forms will be submitted and the licensing inspector will be notified of the results.
