During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on 3/17/2025 and completed on 3/24/2025. The inspection started at 3:57 pm and concluded at 5:50 pm. There was 1 child present with one provider at the time of the inspector's arrival and a second child arrived during the inspection. The point total was 3. The inspector reviewed 3 (all) children?s records and 2 (all) caregiver/household member(s) records on-site on 3/17/2025.
This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/31/2025.
Standard 22.1-289.035-B-3
For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service. Caregiver #1 did not have central registry search results during the inspection and has been employed for over 4 years.
Plan of Correction: The following response was received from the provider:
"Received the updated central registry [results] for...[Staff #1] April 10, 2025 and [Staff #2] on April 18, 2025."
Copies of both Central Registry Results were emailed to the licensing inspector on 3/26/2025.
Standard 8VAC20-790-180-B
(Repeat Violation) The provider is required to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. The last record of attendance the provider had at the time of the inspection was dated October 1, 2024, more than 5 months ago.
Plan of Correction: The following response was received from the provider on 3/26/2025:
"I moved the medicine, got the parents to sign in and out , I put a menu up and I resent for my central registry. I?m working on the rest of the hours."
Standard 8VAC20-790-250-F
(Repeat Violation) Caregivers who work with children are required to obtain a minimum of 16 hours of training related to child safety, child development, health and safety in the family day home environment, and any other required department sponsored training. Caregiver #1 and caregiver #2 each had documentation to demonstrate that a total of 7 hours of annual training were obtained during the 12 month period in 2024 which is 9 hours less than what is required.
Plan of Correction: The following response was received from the provider on 3/26/2025:
"I moved the medicine, got the parents to sign in and out , I put a menu up and I resent for my central registry. I?m working on the rest of the hours."
Standard 8VAC20-790-270-A
(Repeat Violation) The provider is required to store potentially poisonous substances, materials, and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates in areas inaccessible to children. There were 3 potentially poisonous substances located on the sink ledge in the restroom that children utilize while in care. There were 3 potentially poisonous substances located under the sink in an unsecured cabinet in the restroom that children use. There were 2 potentially poisonous substances located on top of a 2 foot tall storage unit next to the toilet. There was a gallon sized bottle of bleach on the floor next to the toilet. All of these items were located in areas accessible to children and each item was labeled "keep out of reach of children" and also had additional warnings on the product label.
Plan of Correction: The following response was received from the provider:
"All Beach, alcohol and peroxide was also put up out of reach of children on 3/24/2025".
Standard 8VAC20-790-400-H
(Repeat Violation) The provider is required to store all medications in a locked place safe locking method that prevents access by children. There was 1 prescription medication and 6 over the counter medications located on a shelf at a height of approximately 5 feet in an unsecured cabinet in the kitchen next to the table where children eat meals and snacks. There were 2 over the counter medications located in an unsecured drawer of a 2 foot tall storage cart next to the toilet in the restroom that children use while in care.
Plan of Correction: The following response was received from the provider :
All medication was moved on 3/24/2025 to a box and in my bedroom in the closet. The door stays locked until every child is gone."
Standard 8VAC20-790-430-B
(Repeat Violation) The provider is required to maintain a record of the dates of the practice drills for one year, divided evenly among the various shifts. The last recorded evacuation drill, which should be practiced monthly, was dated November 2024, more than 4 months ago. The last documented shelter in place drill, which should be conducted two times each year, was dated September of 2023, 16 months ago.
