During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 4/10/25, as a part of the provisional licensure period.The on-site inspection began at 12 noon and ended at 2:05 pm.The inspector reviewed compliance in the areas listed above. There were 35 children present and 7 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-B
The heating system shall be installed to prevent accessibility of children to the system and have appropriate barriers to prevent children from being injured from the heating equipment.

The door to the electric room and the heating system was unlocked and the wires from the unit did not have barriers to prevent access.
Plan of Correction: We will make sure the electrical room is kept locked.
Standard 8VAC20-780-510-E
The center's procedures for administering medication shall Include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. Long-term prescription drug use and over-the- counter medication may be allowed with written authorization from the child's physician and parent.

The authorization form used by the center for emergency medication for child A does not identify the center as having the authorization to administer the medication.
