Inspector Notes

A monitoring inspection was conducted on May 4, 2022 and concluded on May 4, 2022. On May 4, 2022, the licensing inspector was onsite from approximately 10:30 am to 12:00pm. The director assisted with the inspection. There were 18 children present, ranging in ages from three years to five years, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of four staff records, the center did not obtain a central registry finding within 30 days of employment or volunteer service for one staff member.

Evidence:

The record of staff #4, hired 11/05/2021, had documentation of a central registry dated 01/10/2022, exceeding 30 days from employment.
Plan of Correction: Per the Director: Going forward, we will be diligent in following up on the status of background checks with phone calls and documenting all communications to ensure background checks are obtained during the required timeframe.
