Inspector Notes

An unannounced monitoring inspection was conducted this date. There were no children onsite due to a staff planned event and preparation of classrooms for the yearly transitions of age groups. A total of 19 staff onsite today. Staff were observed in their classrooms, preparing their rooms with supplies, cleaning and rearranging the classrooms. Observation of some classrooms made today, review of topical ointments and medications within the classroom was conducted. A total of 4 children's records and 3 new staff records were reviewed, MAT training to include the AuviQ training course was verified. The classrooms were furnished with age appropriate toys, equipment and materials designated for each class. The playground was found with padded surfacing and maintained in good repair. Thank you for your cooperation today. Additional documents were requested for review and will be forwarded by the Center Director to the Licensing Inspector. This inspection occurred approximately between the hours of 10am and 12:45pm.

Based on staff record review, one background check was not completed.
Evidence:
Staff #2 has lived outside of Virginia in the past five years. An out of state criminal background check was not obtained prior to hire.
Plan of Correction: An out of state criminal background check for this staff was completed and forwarded to the Licensing Inspector for review. A copy will be maintained in the record.
