Inspector Notes
An unannounced mandated inspection was conducted on 08/16/2022 from 1:35pm-4:00pm. There were 119 children supervised by 23 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in playing outside, group play, and napping. Areas of non-compliance are identified in the Violation Notice. 7Evidence: Staff #5 did not have results of an out-of-state child abuse and neglect registry, out-of-state sex offender registry check, and out-of-state criminal history check.
Employment date for staff #5 was 4/14/2022.
Evidence:
1.Staff #4 did not have results for a child abuse and neglect central registry check. The center stated that the central registry check was mailed on 10/19/21.
Employment date for staff #4 was 8/9/2021.
Evidence:
1.Staff #1 last tuberculosis screening on file was dated 02/13/2019.
2.Staff #2 last tuberculosis screening on file was dated 03/16/2019.
3.Staff #3 last tuberculosis screening on file was dated 03/23/2019.
Evidence: Staff #3 Daily Health Observation Certificate was expired on 6/28/2022. There was no one else on site with a current Daily Health Observation certification.
Evidence:
1. In the Kittens classroom, a cleaning product was inside an unlocked cabinet.
2. In the Cardinals classroom, a cleaning product was hanging on an art rack above the sink.
3. In the Chicks classroom, a cleaning product was located above a sink adjacent to the diaper changing table.
Evidence: Center management stated tummy time is practiced for each infant unable to turn over by themselves. However, the center does not document tummy time.
Evidence:
Staff #2 and #5 records were missing documentation that two or more references as to character and reputation as well as competency were checked before employment.
Employment date for staff #2 was 3/18/2019. Employment date for staff #5 was 4/14/2022.
