Inspector Notes
An unannounced renewal inspection was conducted on 2/1/2023 from 10:45pm-2:08pm. There were 135 children directly supervised by 20 direct care staff. The physical plant, 7 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in spanish, circle time, and tabletop activites. Hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. 3Evidence: Staff #1 (DOH: 9/27/2021), staff #3 (DOH: 8/6/2019), and staff #5 (DOH: 11/12/2019) record did not contain a central registry check results from a state they have listed as residing in within the previous five years.
Evidence: The record for staff #2 (DOH: 12/15/2022) did not contain documentation of a negative tuberculosis screening.
Evidence: Staff #2 (DOH:12/15/2022), staff #3 (DOH: 8/6/2019), staff #4 (DOH: 6/1/2022), and staff #5 (DOH: 11/12/2019) records were missing documentation that two or more references as to character and reputation as well as competency were checked before employment.
