During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on January 15, 2025 and completed on January 15, 2025, as a part of the licensure period. The on-site inspection began at 9:16am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 114 children present and 22 staff. The inspector reviewed ten children?s records and ten staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289-036-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The center did not request a search of the child abuse and neglect registry and a sex offender registry checks from State A, State B and State C for Agent #1, who is an applicant for licensure and does not work with children at the center.
Plan of Correction: Working on obtaining and completing the required background checks.
Standard 22.1-289.036-A
The center is required to have agents of an applicant for licensure to undergo a background check every five years.

The following background checks were over due for Agent #1: The most recent sworn disclosure was overdue by a year and eight months. The most recent national criminal background check was overdue by a year and seven months. The most recent central registry was overdue by a year and six months.
Plan of Correction: Obtained and completed the sworn disclosure for Agent #1 on 1/15/2025. Will request and obtain the required repeat background checks for Agent #1.
Standard 8VAC20-780-240-A
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #6, who is working with children, has not yet completed this course which is overdue by 102 days.
Plan of Correction: The staff member will complete the training by 1/17/2025.
Standard 8VAC20-780-280-B
REPEAT VIOLATION:

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

In both two year old classrooms, the cleaning solution and sanitizing solution were in an unlocked cabinet, out of reach of children. An antibacterial foam solution with warning labels, ?Keep out of reach of children,? was stored on an open shelf under the sink in the older infant/young toddler classroom, within reach of children.
Plan of Correction: Will remind staff that cleaning solutions must always be stored in a locked cabinet. Will provide training to staff again.
Standard 8VAC20-780-520-C
If diaper ointment or cream is used, it must be inaccessible to children.

Five diaper creams were stored under the diapering area, within reach of children, in the two year old classroom.
Plan of Correction: Will remind and retrain staff that diaper creams must be stored inaccessible to children.
