During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/4/2026 and completed on 3/4/2026, as a part of the licensure period. The on-site inspection began at 9:44am and ended at 11:57am. The inspector reviewed compliance in the areas listed above. There were 120 children present and 20 staff. The inspector reviewed ten children?s records and ten staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #5, employed for more than ten months, did not have documentation of requesting a criminal background check from one state.
Plan of Correction: Staff #5's appointment to request a criminal background check for one state has been scheduled on 3/5/2026.
Standard 8VAC20-780-240-A
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #1 completed the course 111 days past the ninety-day requirement and Staff #4 completed it 14 days past the ninety-day requirement.
Plan of Correction: Will ensure that employees complete the required pre-service course within 90 days of hire.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

The plastic lid of the trash can in the Cardinal classroom was broken, leaving a hole with a sharp edge in the lid.
Plan of Correction: The trash can was replaced with a new one.
