An announced, on-site monitoring inspection was initiated and completed on 11/6/2025. A file review was conducted at the main office of Campagna Center. The on-site inspection began at 10:30am and ended at 11:50am. The inspector reviewed compliance in the areas listed above. 1 staff file was reviewed on site. This inspection included: document review.
