Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including
telephone contacts, documents review, interviews (if applicable) and a virtual tour of the
program (if applicable).

A complaint inspection was initiated by the local agency representative and inspector on December 2, 2021 regarding concerns of staffing and supervision and special care provisions and emergencies Interviews with staff and collaterals were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. There are no violations cited resulting from this inspection.
