Inspector Notes

A focused monitoring inspection was completed on Tuesday, April 11, 2023.There were 46 children present with 9 staff supervising including an administrative staff on site. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff records and interview, the center did not ensure to obtain a copy of the results of a search of child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.,

Evidence: 1. In Staff#1's record (date of hire: 12/22/2022), there was no results of a central registry finding found. 2. In Staff#3's record(date of hire: 01/27/23) there was no results of a central registry finding found. 3. Interview with staff confirmed results from out of state central registry searches were not available for review during the inspection.

Based on review of children's records and interview, in 3 of 5 children's records the center did not ensure to obtained all information for each child enrolled.

Evidence:
1. In Child#1's record one of the emergency contact address was incomplete.
2. In Child#3's record one of the emergency contact address as incomplete.
3. In Child #4's record both of the emergency contacts addresses were incomplete.
4. Interview with staff confirmed the information for children's records wasn't available for review during the inspection.
