Inspector Notes
A monitoring inspection was completed on June 13, 2024 from 1:04 pm until 3:13 pm at the child day center. There were 16 children present with 5 staff supervising including an administrative staff on site. There were observations at the center of the children in care napping, playing with age appropriate equipment, tummy time, sleeping in designated cribs, diapering by staff, bathroom breaks/handwashing, and transitioning from napping. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If there are any questions regarding the inspection please contact me inspector, Tiffany Jones at 757-403-3045.
Violations
4Evidence: In Staff#4's record (date of hire: 12/29/23), the documentation for a negative tuberculosis screening was not available for review.
Interview with staff confirmed the documentation available for review during the inspection.
Evidence: 1.In the Caterpillar 1 group, the staff to children ratio was 1:9 during the inspection. The required ratio for the staff to children is 1:4.
2. Interview with staff confirmed the group was combined while other staff was on break, and the youngest child was 10 months.
3. A review of the electronic device revealed the current staff to child ratio during the inspection.
Evidence: Staff#2 was observed diapering a child in care and returning the child to the table with the other children prior to handwashing.
Staff#2 was observed reaching for another child after toileting and prior to completion of handwashing.
Evidence:
1. In Caterpillar 1 room, the diapering surface was observed with two bottles placed on it.
2. Photographs of the observation during the inspection.
3. Staff#2 completed diapering of one child in care, and did not clean and sanitize after each use.
